Basic Employee Life Insurance
Natus provides all eligible employees with a basic life insurance policy at no cost to you. This benefit can give you peace of mind knowing that your loved ones will not be left with a lot of expenses if you were to die unexpectedly.
The plan provides coverage at 2 times your annual salary, up to a $500,000 maximum.
It is typical for life insurance plans to reduce benefits at ages 65, 70 and 75. However, Natus’ basic employee life insurance plan does not reduce benefits at any age.

Accidental Death & Dismemberment Insurance
Natus provides all eligible employees with an Accidental Death & Dismemberment insurance policy at no cost to you. The plan provides benefits to you following an accidental death or certain bodily injuries. You also have access to Accident Prevention benefits, you can receive the below exams annually .
The plan provides

Voluntary Employee Life Plan
Voluntary Employee Life Plan is a life insurance plan that you can purchase in addition to the basic life. It's a way to expand your existing life insurance coverage if it's insufficient to cover your family's financial needs in the event of your death.
If you are a newly hired employee electing coverage during the first 31 days of employment at Natus, you may purchase 1 to 4 times your annual salary, up to a $500,000 maximum.

The Hartford will automatically approve your voluntary life coverage up to $350,000. For amounts exceeding that limit, you will need to complete an Evidence of Insurability (EOI) form and obtain approval from The Hartford. The Hartford will review the information you submit and notify you of the results. You will pay for the amount exceeding $350,000 only when it is approved by The Hartford.
If you are an existing Natus employee and decide to purchase coverage at annual enrollment* or during a qualifying life event, you will need to complete an EOI form for any requested amount. The Hartford will review the information you submit and notify you of the results.

Review the online Evidence of Insurability (EOI) process for new employees who want to purchase voluntary life coverage for the first time and for employees experiencing a qualifying life event who would like to make changes to their current elections.
Voluntary Spouse Life Insurance
Natus also provides an opportunity for you to purchase additional life insurance for your spouse from The Hartford. The beneficiary will be paid the death benefits in the event of the death of a spouse. You will need to purchase life coverage for yourself first if you want to purchase coverage for your spouse. The total voluntary spouse life amount cannot exceed 100% of your approved voluntary life election amount.

If you are a newly hired employee electing coverage during the first 31 days of employment at Natus, you may purchase for your spouse any multiples of $5,000, up to a $250,000 maximum, but not to exceed 100% of your approved life insurance amount. The Hartford will automatically approve your spouse life coverage up to $50,000. For amounts exceeding that limit, you will need to complete an EOI form and obtain approval from The Hartford. The Hartford will review the information you submit and notify you of the results. You will pay for the amount exceeding $50,000 only when it is approved by The Hartford.

If you are an existing Natus employee and decide to purchase coverage at annual enrollment* or during a qualifying life event, you will need to complete an EOI form for any requested amount. The Hartford will review the information you submit and notify you of the results.

Voluntary Child Life Insurance
Natus also provides an opportunity for you to purchase additional life insurance for your children from The Hartford. The beneficiary will be paid the death benefits in the event of the death of a dependent child. You need to purchase life coverage for yourself first if you want to purchase coverage for your children.

If you are a newly hired employee electing coverage during the first 31 days of employment at Natus, you may purchase for your dependent child or children up to age 26 life insurance coverage in an amount of $10,000.
Employees may purchase voluntary child life insurance for dependents up to age 26 and the life insurance benefit is $10,000.
Basic Short Term Disability Insurance
Short Term Disability Plan pays you a weekly benefit if you are unable to work due to an off-the-job sickness or injury. You can use the money however you choose, such as for monthly recurring household expenses.
Natus offers this benefit at no cost to you. The plan provides 60% of weekly earnings up to a $3,000 weekly benefit maximum, beginning on the 8th day of your sickness or injury. The maximum number of weeks you can receive benefits while you're disabled is 12 weeks. The weekly benefit may be reduced or offset by other sources of income.

Voluntary Short Term Disability Insurance
Natus provides an opportunity for you to purchase additional short term disability coverage to help protect more of your income if you’re ever unable to work. Natus' Voluntary Short Term Disability plan is offered through The Hartford. You may elect the plan and pay your premiums with after-tax dollars through payroll deductions.

You are already enrolled in the basic Short Term Disability plan provided by Natus at no cost to you. The basic Short Term Disability plan will replace 60% of your weekly earnings up to a $3,000 weekly benefit maximum, if you are unable to work.
If you decide to purchase the voluntary Short Term Disability plan, the combined basic and voluntary plan will replace 70% of your weekly earnings up to a $3,500 weekly benefit maximum. The maximum number of weeks you can receive benefits while you're disabled is 12 weeks. The weekly benefit may be reduced or offset by other sources of income.
If you decline to purchase the voluntary Short Term Disability plan when you first become eligible, you will be required to complete an EOI form if you want to purchase it at later time. The Hartford will review the information you submit and notify you of the results.

Basic Long Term Disability Insurance
Long-Term Disability Plan provides benefits to replace part of your income if a disability keeps you out of work for a long period of time. Similar to Short Term Disability, the plan allows you to use the money however you choose.
Natus offers this benefit at no cost to you. The plan provides 50% of monthly earnings up to a $8,500 monthly benefit maximum, beginning 90 days from the date of disability. The monthly benefit may be reduced or offset by other sources of income.
